Advocate Trinity Hospital is a faith based health system providing the best health outcomes and building lifelong relationships with those we serve. Advocate Trinity Hospital has provided the south side of Chicago with quality health care for more than 110 years. Now part of Advocate Aurora Health, an integrated health care system formed in 2018 from the merger of Advocate Health Care and Aurora Health Care, the hospital continues as a vital part of the community, serving the health needs of individuals, families and communities with a holistic philosophy of care. Each year Trinity serves more than 90,000 patients with a full range of inpatient and outpatient care including emergency services, surgery, heart health, diabetes treatment and cancer care. Nearly 85% of the population in Trinity’s primary service area is African American and the hospital’s services and community health outreach efforts reflect a commitment to respond the community’s most pressing health needs. In 2016, the hospital identified that diabetes was a pressing health concern through it community needs assessment. To address this need, the hospital developed a three-year implementation plan to prevent and reduce the incidence of diabetes by implementing the Healthy Living program and National Diabetes Prevention Program.
