Legal Council for Health Justice works to decrease health disparities in Chicago by addressing social determinants of health using a Medical-Legal Partnership model. We establish mutually effective community partnerships with trusted Healthcare and Social Service providers to provide health-promoting legal services to communities historically excluded from healthcare access. An NHC member will be the perfect fit to support our efforts in community outreach and building healthy Medical/Legal Partnerships. The NHC members would do this by doing the following:
• create and maintain a resource bank to be used by clients and healthcare providers. Such resources can expand the capacity of the MLP by helping patients identify legal issues that could be affecting their access to health care.
• Create educational programs that can be used to engage clients in new and expanding partnerships
• Create tools to help assess the needs and assets of our client population.
• Create tools to encourage and measure client feedback about our services and how to improve
• Build our capacity to create meaningful opportunities for engagement with the client populations that we serve.
