Position Summary
About the Position: The Sexual and Reproductive Health Coordinator will support people experiencing homelessness in accessing comprehensive sexual and reproductive health services, including but not limited to pregnancy testing, prenatal care, HIV, hepatitis, STI testing, prevention, and treatment. Specific areas of focus will include supporting Whole Person Integrated Care's Ending the Epidemic (ETE) initiative, focused on HIV, Hepatitis C, and sexually transmitted infections, as well as decreasing the risk on congenital syphilis and improving birth outcomes. The NHC Member will work with a multidisciplinary care team, including nurses, health workers, peers, social workers, and medical providers.

This AmeriCorps position will be based at a low-barrier and transitional care urgent care clinic. This position will be a key part of efforts to meet goals for the Ending the Epidemic Initiative, as well as improving perinatal services. This position will work with multiple sections of the Department of Public Health, including Population Health, Maternal Child Adolescent Health, and HIV Health Services.
About the Organization: SFDPH Whole-Person Integrated Care serves to provide care to people experiencing homeless by 1) connecting to transitional primary care 2) re-/connecting to social services 3) supporting client’s towards becoming “housing ready” for their transition from shelter health to their next destination, ideally their own permanent, supportive housing with established primary medical care.
Shelter Health is one of 8 social medicine programs under the Whole Person Integrate Care umbrella. This position would be a vital part of care coordination, stabilization, and wrap-around services to adults experiencing homelessness.
Major Duties and Responsibilities
Duties and responsibilities may include:
- Identifying patients who are at-risk to assess eligibility for various services
- Referring patients to other programs and community services
- Implementing patient engagement strategies to improve outcomes
- Participating in meetings and conferences
- Conducting program quality improvement and data analysis

Knowledge Required for the Position
Supervision
- The supervisor assigns service activities, advises on changes in procedures, and is available for assistance when required

- Completed projects are reviewed very generally to determine that objectives have been met and are in compliance with policies and regulations.

- Written and oral guides provide specific instructions for doing service
Complexity
- Member has to develop, analyze, or evaluate information before the service position can progress
- The member must develop new techniques, methods, or approaches to complete service position assignments
Special Considerations
The NHC Member will:
- Serve Monday - Friday, 8:00AM - 5:00PM
- Serve approximately 36-40 hours per week at their Host Site
- Serve primarily in-person
- May support evening or Saturday clinics with coordination from Host Site and Program Supervisor
