Position Summary

The Family Practice and Counseling Network (FPCN/the Network), a program of Resources for Human Development (RHD) and medical home practicing site of Rep. Tarik Khan, is a network of 13 federally qualified community health centers (FQHCs), inclusive of 6 school-based clinics in partnership with the School District of Philadelphia; a convenient care clinic, Nourish our Wellness (NoW) clinic in the Island Ave ShopRite in SW Philadelphia; as well as a mobile unit, Wellness on Wheels (WoW); that uses a nurse-managed, integrated model of health care throughout vulnerable communities of Greater Philadelphia and surrounding counties. Our 31- year history of providing comprehensive health care services, inclusive of primary care, behavioral health and dental health care to the underserved, is rooted in a “wrap around” model that includes a wide array of services such as chronic care management, integrated behavioral health, reproductive health and education, on-site physical therapy and mindfulness, adjunct specialty care (cardiology, podiatry and optometry) and peri-natal health services caring for communities holistically, mind, body and spirit. Through community partnerships and serving as a voice and stakeholder for our trusted village, we embark on in service with the mission that “quality healthcare is a right, not a privilege.”

Major Duties and Responsibilities

Providing outreach to agencies to build awareness. Actively participating in health campaigns by monitoring health issue trends. Creating and developing communications related to outreach campaigns. Developing and providing health education for the community. Gathering data on critical issues impacting the community.
