Position Summary
The Wright Center is initiating a program wherein a Community Health Worker will work directly under and with the Director of Public Affairs and the Patient Community Engagement(PCE) Board. The community health worker (CHW) will be responsible for helping to expand The Wright Center's mission of improving the health and welfare in communities . This will be accomplished by the CHW working directly with the Director of Public Affairs and PCE board in the community and clinic settings. This initiative is vital to removing barriers and access to quality, affordable healthcare by addressing SDOH disparities. Currently, The Wright Center does not have a CHW working in this capacity and we are looking to initiate this service line with the goal of having it grow and expand into every community that we serve.
Major Duties and Responsibilities
The PCE CHW will work closely with the Director of Public Affairs, The Director of Patient Engagement, and the PCE Board of Directors, as well as medical providers, to provide resources to the community and patients that we serve who are struggling with food, shelter, clothing, obtaining vaccinations, dental care, medical care, etc. by directly connecting them with social service agencies. The PCE CHW will also work with our Driving Better Health Team to assist in providing health screenings/services directly in the community. Their role will be to close the
gap on the disparities in the SDOH'S so that the community members have access to vital medical, social, and other vital services.
