Position Summary
This position will work with the Director of the Geriatric Service Line and Team members. The NHC member will be working with patients ages 60 years of age and older to get the resources they need to have the best quality of life. The member will also be working with caregivers to assist with emotional support and resources.
Major Duties and Responsibilities
1. Patient calls related to SDOH - senior isolation questions which are "How often do you feel lack of companionship? how often do you feel left out? How often do you feel isolated from others?" Then would make referrals to community partners such as the United Way social isolation project.
2. Working with the geriatric team on resources for patients and caregivers - this would include pre-visit planning and post-visit planning for resources and support services. Planning education groups.
Establishing help with housing - not only for homeless elders but transitions to housing as their care needs change. Working with the doctor and the nurses and the caregiver as a team to be sure the patient is safe in their home environment. providing assistance to get DME equipment from a local DME provider that supplies re-furbished DME for those who do not have insurance coverage. Home evaluations to see if the patient requires home medical visits
3. Assisted Living and Home health - Gather different brochures for patients and caregivers to allow them to explore care options
4. Assisting patients to get into programs such as Adult Day services / Senior Centers- gather brochures and have these ready for patients and caregivers to have options
5. Finances and costs associated with older adults work with Area Agency on Aging to assist patients and caregivers with the waiver program and the caregiver support programs
6. Support services for the caregivers - connect caregivers to resources for respite as appropriate
