Outreach Coordinator

Position Summary

We have designed this position to provide service and bring more awareness of the program within the community
Tasks are not limited to the following-
• Assisting in community events (health fairs, lifestyle classes, etc.)
• Expanding partnerships throughout the community (educating agencies about Healthy Start services, providing information about resources in our area, etc)
• Assist with CI & R Outreach and Initial Contacts, picking up and mailing of screens
• Update the Resource list for St Johns and Duval county as well as add any new resources in the area.
• Assist the Healthy Start Care Coordinator in bringing curriculum to St Gerards school for pregnant and teen mothers given bi-weekly throughout the school year and other activities as assigned.
• Participate in monthly supervisions (according to our contractual requirements)

Major Duties and Responsibilities

  • Providing outreach to agencies to build awareness
  • Attending trainings and/or community events
  • Maintaining educational materials in waiting/examination rooms
  • Identifying new sites to provide resources and materials
  • Teaching classes/workshops in-person and remote on health

Characteristics of an Ideal Candidate

Self-starter, Flexible, Comfort speaking with others, individually or in groups, Community engagement/outreach skills, Able to communicate effectively with individuals or diverse backgrounds and identities

Knowledge Required for the Position

Skill with Microsoft Office or other software for a variety of data processing operations involving a range of problem solving, record keeping, correspondence, and service tracking options.

Supervision

Member uses initiative in carrying out recurring assignments following set procedures, independently, The supervisor assigns service activities, advises on changes in procedures, and is available for assistance when required

Review

The service activities are reviewed for accuracy through spot checks, through complaints from customers, and through observation of the member at service., Completed projects are reviewed very generally to determine that objectives have been met and are in compliance with policies and regulations.

Member has little or no choice about how to perform the service position, The service position assignments vary significantly or often involve unusual circumstances and incomplete or conflicting data, The member must develop new techniques, methods, or approaches to complete service position assignments

Special Considerations

Unusual service hours, involving evenings, weekends, or long shifts, Personal vehicle required, Significant travel (more than 25%), Position offers flexible hours/service formats for members who may need them due to childcare commitments, probation/parole requirements, or other obligations.
