Position Summary

About the Organization: SFDPH Whole-Person Integrated Care serves to provide care to people experiencing homeless by 1) connecting to transitional primary care 2) re-/connecting to social services 3) supporting client’s towards becoming “housing ready” for their transition from shelter health to their next destination, ideally their own permanent, supportive housing with established primary medical care.  

Shelter Health is one of 8 social medicine programs under the Whole Person Integrate Care umbrella. This position would be a vital part of care coordination, stabilization, and wrap-around services to adults experiencing homelessness.

About the Position:

The Substance Use Care Coordinator will support people experiencing homelessness in linkage to substance use treatment to reduce the risk of overdose and improve their health. The NHC Member will support the INSPIRE Clinic, a multidisciplinary care team, including nurses, health workers, peers, social workers, and medical providers, to offer treatment to people with stimulant use disorder using a model called Contingency Management. The NHC Member will also help people with opioid use disorder start and stay on medication for addiction treatment through patient navigation, outreach, and linkage to services. Additionally, the NHC Member will support the effort of the Post Overdose Engagement Team (POET) to decrease the risk of subsequent overdose for people who have had a prior non-fatal overdose.

The NHC Member will be based at the low-barrier and transitional care urgent care clinic, the Maria X Martinez Health Resource Center. This position will be a key part of efforts to meet our goals for the SFDPH Coordinated Overdose Response and Engagement Initiative. This position will work with multiple sections of the Department of Public Health, including Population Health, Behavioral Health Services, Pharmacy Services, and with multiple community based organization partners.

Major Duties and Responsibilities

Duties and responsibilities may include:

  • Referring patients to other programs and community services
  • Tracking patients to confirm program enrollment and engagement
  • Implementing patient engagement strategies to improve outcomes
  • Educating patients on medical interventions and prevention
  • Conducting program quality improvement and data analysis
  • Participating in the twice weekly INSPIRE clinic to support patient attendance, flow, and follow up

Supervision

Special Considerations

The NHC Member will:

  • Serve Monday - Friday, 8:00AM - 5:00PM
  • Serve approximately 36-40 hours per week at their Host Site
  • Serve primarily in-person
