Position Summary
About the Position: The Shelter Health Public Health Educator will support adults experiencing homelessness and living in shelters and will engage and promote health education topics, such as sexual health, to this community. The NHC Member will collaborate with pre-existing patients and community-based organizations. The NHC Member will also create healthcare and wellness educational materials, provide health counseling and coaching, and organize documentation and educational materials. The NHC Member will fill a much-needed gap in providing educational materials and resources related to public health services.
About the Organization: SFDPH Whole-Person Integrated Care serves to provide care to people experiencing homeless by 1) connecting to transitional primary care 2) re-/connecting to social services 3) supporting client’s towards becoming “housing ready” for their transition from shelter health to their next destination, ideally their own permanent, supportive housing with established primary medical care.
Shelter Health is one of 8 social medicine programs under the Whole Person Integrate Care umbrella. This position would be a vital part of care coordination, stabilization, and wrap-around services to adults experiencing homelessness.
Major Duties and Responsibilities
Duties and responsibilities may include:
- Engaging in outreach and promoting health education topics and healthcare services
- Collaborating with pre-existing patients and partner organizations
- Creating healthcare and wellness educational materials
- Providing health counseling and coaching
- Maintain a resource binder for shelter clinics and guests
- Attending overdose prevention and equity learning workshops
- Supporting health and racial equity projects

- Comfort speaking with others, individually or in groups

- Comfortable working with a diverse population of staff and patients (i.e. POC, LGBTQIA+ communities, Variety of age groups, collaborating with multidisciplinary teams and being flexible)

Supervision
Review
- Completed projects are reviewed very generally to determine that objectives have been met and are in compliance with policies and regulations.
Guidelines
- Written and oral guides provide specific instructions for doing service
- Guidelines are generally applicable, but the member independently makes adaptations in dealing with problems and unusual situations
Complexity

Special Considerations
The NHC Member will:
- Serve Monday - Friday, 8:00AM - 5:00PM
- Serve approximately 36-40 hours per week at their Host Site
- Serve primarily in-person
