Position Summary
The NHC member will play a critical role in expanding the services that JASMYN and its current staff can offer to address the health challenges and disparities of LGBT youth. They will play a critical role in supporting homeless and unstably housed youth, youth experiencing substance use, youth at high risk for HIV and youth living with HIV. The NHC member will assist with the ACCESS Program as well as provide Health Education:
JASMYN’s ACCESS Program (Accessing Client-Centered Essential Support Services)
- The member will assist with the staffing of JASMYN’s ACCESS program that provides crisis intervention, health screenings, risk behavior assessments, HIV testing and counseling safety planning and enrollment into JASMYN’s intensive case management services for youth experiencing homelessness, substance use, legal and mental health issues. The member will work one on one providing direct services to JASMYN youth during ACCESS. They will work with supervisors to create outcome measures for ACCESS that will help streamline internal enrollments into case management, create a community resource guide to improve awareness of local resources available in the community and This aspect will focus on developing working relationships with youth accessing JASMYN and providing individual support through health assessments, membership enrollments, risk screening and resource linkage.
Health Education
- JASMYN’s youth center is open most weekday afternoons for LGBTQ youth and their friends to find support and positive youth development activities. The member will create capacity for expanded health education in the youth center through the development and implementation of workshops in topics such as healthy eating and fitness, smoking cessation, healthy body image, gender and sexual identity, substance use and abuse topics, healthy relationships, and sexual health. While some of these sessions will be formally scheduled, it is anticipated that many of these short educational sessions using conversations and online tools will be presented in “teachable moments”, when the topics are most relevant to the immediate needs and questions that arise in the friendly conversation of the youth center. This aspect of the position will focus on empowering youth to make smart choices about their health and lead healthier lives.
Major Duties and Responsibilities
- Practicing intake and referrals of patients in need
- Identifying patients who are at-risk to assess eligibility for various services
- Referring patients to other programs and community services
- Tracking patients to confirm program enrollment and engagement
- Providing follow-up and case management to deliver assistance and information
- Implementing patient engagement strategies to improve outcomes
- Maintaining professional boundaries and respecting patient confidentiality
- Coordinating health and wellness activities including classes and groups
- Documenting interactions with patients in an electronic health record
- Attending and participating in community or host site events
- Creating social media content to promote the programs
- Collaborating with supervisors and other leadership
- Screening persons admitted for referral to behavioral health services
- Providing social service navigation
- Participating in meetings and conferences
Characteristics of an Ideal Candidate
- Enjoys working with teens and young adults in a diverse and multi-cultural environment.
- Has the ability to relate to teens and young adults in an accepting, positive and healthy manner.
- A person who is energetic, outgoing, patient, compassionate, and able to stay focused and organized. A person who is flexible, adaptable and has strong initiative.
- A member with a BA or BS degree in social work or a related field such as education, public health, social welfare or psychology is preferred.
- Critical skills include the ability to coordinate multiple responsibilities, collaborate with community partners, and conduct resource brokerage and referral.
- Experience with adolescents and conducting health education, and familiarity and sensitivity in dealing with the unique needs of LGBTQ youth will be essential.
- Members must pass a criminal background check.
- LGBTQ cultural competency, comfortable and confident in taking with youth about sexuality, HIV/AIDS, and STI’s, open mind, flexibility, great initiative, positive attitude, teamwork and a sense of humor.
Knowledge Required for the Position
Knowledge of AmeriCorps/Health Corps member requirements
Skill with Microsoft Office or other software for a variety of data processing operations involving a range of problem solving, record keeping, correspondence, and service tracking options
Supervision
Member uses initiative in carrying out recurring assignments following set procedures, independently.
The supervisor assigns service activities, advises on changes in procedures, and is available for assistance when required.
Review
The service activities are reviewed for accuracy through spot checks, through complaints from customers, and through observation of the Member at service.
The Member independently plans and carries out the projects and selects the approaches and methods to be used in solving problems.
Completed projects are reviewed very generally to determine that objectives have been met and are in compliance with policies and regulations.
Guidelines
Written and oral guides provide specific instructions for doing service.
Guidelines are generally applicable, but the Member independently makes adaptations in dealing with problems and unusual situations.
Member must use considerable judgment in adapting current or developing new guidance.
Complexity
The service position assignments vary significantly or often involve unusual circumstances and incomplete or conflicting data.
In making decisions, the Member is often required to depart from past approaches and to extend traditional techniques.
The Member must develop new techniques, methods, or approaches to complete service position assignments.
